A new report from American Principles Project reveals that the transgender surgery industry has exploded into a massive revenue-generating system, surpassing billions of dollars in 2023 and projected to grow exponentially by 2030. Concerns arise over the industry's financial incentives and the potential consequences for individuals undergoing these procedures.
Driven by the increasing number of individuals identifying as transgender, the demand for transgender surgeries and related treatments has surged, leading to a booming multi-billion dollar industry. According to a comprehensive report by conservative think tank American Principles Project (APP), the total revenues for transgender drugs and surgeries in 2023 were estimated to surpass $4.4 billion. This staggering figure is projected to exceed $7.8 billion by 2030, highlighting the rapid growth of the industry.
Terry Schilling, President of APP, expressed concern about the industry's expansion and its implications for children. "There are obvious physical and psychological consequences to these procedures, and we wanted to understand the driving force behind their growing popularity," Schilling said. "Our investigation uncovered a significant financial incentive that may be fueling the industry's expansion."

The APP report sheds light on the potential financial rewards for providers of transgender surgeries. Healthcare facilities such as Cedars Sinai, the Regents of the University of Michigan, and the Mount Sinai Health System have reportedly generated over $100 million in revenue from these procedures in 2022 alone. Pharmaceutical corporations, like Pfizer and AbbVie, have also benefited immensely. Pfizer earned $74 million while AbbVie raked in $51 million from transgender medications.
According to the report, lifelong use of cross-sex hormones can amount to expenses of up to $300,000, while surgical transitions can cost individuals upwards of $150,000. Additionally, the potentially harmful side effects of transition surgery, such as cancer, nerve damage, chronic pain, sexual dysfunction, and mental health issues, can lead to further medical expenses and lifelong complications.

Despite these known risks, the industry continues to promote transition surgeries, raising concerns about the motives of those involved. "We like to believe that most individuals have good intentions and do not wish to harm others," Schilling said. "However, when we realize the magnitude of financial incentives at play, it becomes clearer why people may be encouraged to undergo these potentially harmful procedures."
The APP report estimates that 1.6 million Americans now identify as transgender, with 300,000 individuals falling within the 13-17 age range. Pro-gender transition activists have projected that the industry could surpass $200 billion per year, surpassing the film industry in revenue.
The APP report warns that the industry may face legal repercussions as more individuals regret and experience health issues resulting from their transitions. Detransitioners' public stories and potential lawsuits could lead to a reckoning for the industry, similar to the decline of lobotomy practices.
"Given the lucrative nature of this industry, medical practitioners will likely continue to recommend and administer these drugs and procedures for as long as they feel safe from liability," APP said in a press release.
The report urges policymakers to recognize the urgency of addressing the risks and financial incentives associated with the transgender surgery industry. Schilling emphasizes the need for government action to prevent further harm to individuals and families.
"It is vitally important that the American people see the reality of what is happening: the damage being done by these practices as well as the financial incentives which keep the industry going despite the consequences," Schilling said. "Government action is necessary, and it cannot come a moment too soon."
